Determine Killip class in acute MI: I–IV by symptoms with 30-day mortality from GUSTO-I.
Killip 1967 classification is a clinical heart failure assessment in acute MI based on bedside exam at presentation. Class I: no signs of HF (30-day mortality ~5%). Class II: rales over lower lung fields, S3 or elevated JVP (~14%). Class III: acute pulmonary oedema (~32%). Class IV: cardiogenic shock (~58%). It feeds the GRACE 2.0 score and shapes management – from conservative in I to emergency revascularisation and mechanical support in IV.
