Diabetic polyneuropathy
Adjunct
Adjunct therapy for painful diabetic polyneuropathy. AEMPS approves Thioctacid 600 in confirmed disease: IV 600 mg/day for 2–4 weeks, then oral 600 mg/day for up to 6 months. The ALADIN/NATHAN meta-analysis showed moderate reduction of pain and paraesthesia vs placebo. EFNS positions the drug as adjunctive, not first-line: first-line options are gabapentin/pregabalin or duloxetine. If pain is severe or ALA does not work after 4–6 weeks, switching therapy with an endocrinologist or neurologist is appropriate.
