Encodes full-length SARS-CoV-2 spike protein in lipid nanoparticles. After injection, maternal cells briefly synthesize spike, triggering IgG antibodies and T-cell response. mRNA degrades within days, does not integrate into DNA. 70-90% effectiveness against hospitalization in first 6 months, lower against infection.
Indications
A
COVID-19 prevention
First line
Prevention of severe COVID-19 in individuals ≥6 months. Per CDC ACIP 2024 and , an annual dose of the current composition is recommended for risk groups: age >65, immunocompromised, pregnant, healthcare workers. Dose 0.3 mL intramuscular.
Active maternal immunization against COVID-19. CDC ACIP 2024 and SEGO 2023 recommend any trimester. No increased risk of miscarriage, preterm birth, or birth defects (Lancet 2022 meta-analysis, n>250,000).
Comirnaty during breastfeeding. CDC ACIP 2024 and AEP CAV 2024 – fully compatible. mRNA and lipid nanoparticles do not transfer to milk in significant amounts. Spike antibodies pass into milk and provide passive infant protection.
Reference information, not a clinical decision. Discuss feeding pauses or changes with your physician or an IBCLC.
