The term 'dyscirculatory encephalopathy' is absent from international neurology and from ICD-11. Mexidol for cognitive impairment and chronic cerebral ischemia is not mentioned in AAN or ESO recommendations. Evidence-based management of vascular cognitive dysfunction relies on vascular risk factor control, physical activity, and cognitive training.

Ischemic stroke

Not recommended

In international guidelines for acute ischemic stroke management (AHA/ASA 2019, ESO 2021), Mexidol and other 'neuroprotectors' are not mentioned. Proven acute therapies include systemic thrombolysis with tissue plasminogen activator within 4.5 hours of symptom onset and mechanical thrombectomy within 6–24 hours in selected patients.

Ethylmethylhydroxypyridine succinate is widely used in Russian neurology and cardiology under the brand names Mexidol, Mexicor, Mexiprim, and Neurox. It is registered in the ГРЛС and included in several Russian Ministry of Health clinical guidelines, creating divergence from international approaches. The molecule is not described as a therapeutic agent in international pharmacological literature.
