Vascular-strengthening effect is proven for oral use (chronic venous insufficiency). Topical effects on dark eye circles are confirmed only by small uncontrolled studies.
Troxerutin (Trihydroxyethylrutin) is a semi-synthetic flavonoid derived from rutin (a flavonoid from buckwheat and Sophora). In pharmacology it is used for chronic venous insufficiency and haemorrhoids (Troxevasin, Venoruton). In cosmetics – claimed capillary strengthening. Mechanism. Lowers capillary permeability, inhibits hyaluronidase and elastase. In eye-area skin capillaries – claimed reduction in visibility of venous congestion (dark circles) and puffiness. Antioxidant effects in vitro. Where applied. Eye creams and serums, products for dark circles and puffiness, products for couperose skin. In Spain – in Sesderma Factor G Renew, Mesoestetic Eye Contour, Endocare Tensage lines. Evidence base. Oral troxerutin in chronic venous insufficiency has moderate evidence (Cochrane 2015: minor symptom improvement). Topical use in cosmetics – RCTs with 'dark circles' as an endpoint are absent. The effect is attributed to oral flavonoid therapy, not topical use. Safety. Sensitises rarely, non-comedogenic. Rare photodermatitis and contact allergy possible (1-2% of patients). Pregnancy – use with caution. Oral troxerutin in the first trimester is not recommended (theoretical teratogenic risk per AEMPS). Topical at cosmetic concentrations is minimally absorbed, but pregnant women with dark circles should pick caffeine and cooling instead. Realistic expectations. Eye dark circles have three causes: pigmentary (hereditary), vascular (visible veins), and tear-trough shadows (anatomy). Topical troxerutin works only on the vascular component, and only on in vitro grounds. Best treatments are filler correction, vascular laser, and concealer makeup.
