The Mifflin-St Jeor formula estimates basal metabolic rate (BMR) and total daily energy expenditure (TDEE) in adults. Developed by Mifflin et al. (Am J Clin Nutr, 1990) in 498 patients across weight categories. Endorsed by Academy of Nutrition and Dietetics 2014 as the most accurate among predictive equations, superior to Harris-Benedict and WHO. BMR formula. Men: BMR = 10 × weight (kg) + 6.25 × height (cm) – 5 × age – 5. Wait, +5 for men, women minus 161. Men: BMR = 10W + 6.25H – 5A + 5. Women: BMR = 10W + 6.25H – 5A – 161. TDEE via activity factor. Sedentary (no exercise) – BMR × 1.2. Light (1-3 sessions/week) – ×1.375. Moderate (3-5 sessions) – ×1.55. High (6-7 sessions) – ×1.725. Extreme (heavy labor or 2 sessions/day) – ×1.9. Clinical use. Caloric prescription in obesity (TDEE – 500-750 kcal for 0.5-1 kg/week loss), undernutrition (TDEE + 500-750 kcal for gain), postoperative and ICU patients (with stress factor 1.2-1.5). In athletes – basis for macronutrient calculation and nutrition periodization. Limitations. Accuracy ±10% even in healthy adults, ±15-20% in obesity, T2DM, hypothyroidism. After age 65 BMR is 5-10% below predicted due to sarcopenia. Not applied in pregnancy or lactation – use separate increments (+300 kcal in 2nd trimester, +500 in 3rd, +500 during lactation). Gold standard – indirect calorimetry, available only in research and ICU settings.
