Dose and concentration converter

Updated · Methodology

Percent here means weight per volume: 1% = 1 g in 100 mL = 10 mg/mL. A 1:1000 ratio = 1 g in 1000 mL = 1 mg/mL

About the conversion

Use this converter when the order and the label use different units: the order says mEq while the vial says millilitres and percent, the prescription says IU while the box says micrograms, or the dose is in milligrams while you hold a dropper bottle or a syringe marked in millilitres. The figures for vials, drops, oral solutions and heparins come from Spanish AEMPS labels. Drops and heparins name their brands and link to their ficha técnica, and for calcium chloride and magnesium sulfate the US and UK strengths are listed next to them.

With vials and vitamins, most mistakes happen in three places. In Spain 10% calcium chloride is the hexahydrate, while in the US and UK it is the dihydrate, which carries one and a half times more calcium. Vitamin E converts from IU to mg differently for the natural and the synthetic form. Calcifediol is not converted to IU at all, as its label states.

With drops and syringes, mistakes happen where one substance comes in different packs. One Adolonta pump is 12.5 mg of tramadol, the same as 5 drops of Tramadol Viatris or Tramadol VIR. Oramorph comes as 2 and 20 mg/mL. Vitamina D3 Fresenius Kabi gives 400 IU per drop, Deltius and similar drops give 200. The Fragmin 2,500 IU/mL vial is ten times weaker than the 25,000 IU/mL syringes. Drops from different makers are not interchangeable, and anti-Xa IU of different heparins are not converted into each other.

The converter only changes units and does not choose a dose. Before you give a dose, check the strength on the label of the vial, bottle or syringe in your hand.
