Asymptomatic hyperuricemia
Elevated serum uric acid without clinical gout or nephrolithiasis. Per ACR 2020 and EULAR 2017, routine urate-lowering therapy is not indicated in this setting.

The drug is promoted for these uses outside international guidelines. Each entry below is analyzed against AEMPS, FDA, EMA, Cochrane and major RCTs.
- FAllopurinolNot recommended · Xanthine oxidase inhibitorsPer ACR 2020 and EULAR 2017, routine urate-lowering therapy in asymptomatic hyperuricemia is not indicated. ACR 2020 conditionally recommends against starting therapy even with concurrent CKD or urolithiasis, and in the CKD-FIX trial allopurinol did not slow CKD progression. Treating an asymptomatic patient carries real harm: severe cutaneous reactions (SJS/TEN, DRESS) and allopurinol hypersensitivity syndrome, more common in HLA-B*5801 carriers. Zero proven benefit against documented harm is why allopurinol is not given routinely here. The widespread practice of prescribing allopurinol 'for high uric acid on labs' without gouty symptoms lacks an evidence base.
- FFebuxostatNot recommended · Selective xanthine oxidase inhibitorFebuxostat is actively prescribed to patients with asymptomatic hyperuricemia without gout or nephrolithiasis for anti-aging uric acid reduction. ACR 2020 and EULAR 2023 recommend against treating asymptomatic hyperuricemia in patients without gout flares or urate nephropathy. CARES 2018 (White) in gout patients with cardiovascular disease showed increased cardiovascular mortality versus allopurinol, prompting an FDA boxed warning 2019 on increased mortality. Use in asymptomatic patients is unjustified.
How to read tiers A–F
Evidence tiers belong to the drug-indication pair, not to the drug as a whole: the same substance can be A for one condition and F for another.
- AStrong evidence – first or second line in international guidelines
- BRecommended in selected cases
- CStudy data contradict each other
- DBenefit not proven, no documented harm
- FDocumented harm, or risk without benefit
