Immunomodulatory drug (IMiD), thalidomide derivative

Binds cereblon (CRBN) in the E3 ubiquitin ligase complex, leading to ubiquitination and degradation of transcription factors IKZF1 (Ikaros) and IKZF3 (Aiolos). This causes direct myeloma cell apoptosis, T- and NK-cell activation, and tumor microenvironment angiogenesis inhibition. Compared with thalidomide, greater myeloma activity and less neurotoxicity, but comparable teratogenic risk.
First line
Foundation component of multiple myeloma therapy per IMWG 2014 and ESMO 2024. In newly diagnosed patients, lenalidomide is in triplet and quadruplet induction (VRd: bortezomib + lenalidomide + dexamethasone; D-VRd: with daratumumab). Lenalidomide maintenance 10 mg for 21 days of 28 after autologous transplant doubles progression-free survival (CALGB 100104). In relapsed myeloma: combinations with carfilzomib, elotuzumab, ixazomib, daratumumab.
Foundation therapy within induction and maintenance regimens.

Not recommended
Lenalidomide is an immunomodulatory drug, a thalidomide derivative. It is prescribed for multiple myeloma, myelodysplastic syndrome, mantle cell lymphoma, and follicular lymphoma (AEMPS Ficha Técnica, US REMS program). In alt-anti-aging circles, lenalidomide is occasionally promoted as an immune modulator for anti-aging. In healthy adults, no clinical studies of such use exist, and such prescription is extremely dangerous. The drug has a catastrophic risk profile: teratogenicity (thalidomide derivative, severe birth defects, boxed warning, mandatory REMS program with dual contraception), severe myelosuppression (neutropenia, thrombocytopenia), thromboembolic events (deep vein thrombosis, pulmonary embolism), and secondary malignancies. If lenalidomide was recommended to a healthy person, seek a second opinion immediately; this prescription is life-threatening.

Boxed warning
FDA boxed warning: embryo-fetal toxicity (teratogenicity – cannot be taken in pregnancy or by men without reliable contraception – transmits via semen); thromboembolic events (increased VTE risk); severe myelosuppression (neutropenia, thrombocytopenia).
Absolute contraindication. Women of reproductive age: two contraception forms from 4 weeks before start, during therapy, and 4 weeks after. Men: barrier method from start and 4 weeks after.
Contraindicated during lactation.
