Marketing claims without evidence base

The drug is promoted for these uses outside international guidelines. Each entry below is analyzed against AEMPS, FDA, EMA, Cochrane and major RCTs.

D

Stress and anxiety

Not recommended

Pregnenolone is a steroid hormone that the body uses as a precursor to other hormones. It is sold as a supplement for stress and anxiety, marketed for calm and mental clarity. Convincing benefit for this use has not been shown: there is no solid evidence that it eases anxiety. For stress and anxiety, other approaches help – psychotherapy (cognitive behavioral therapy first of all), regular sleep and physical activity, and, when symptoms are pronounced, medication prescribed by a doctor.

ICD-10: F41.9 · ICD-11: 6B0Z

F

“Hormonal balance” (marketing claim)

Not recommended

Pregnenolone is sold as the «root of all hormones» with promises to «balance the whole endocrine profile». The Endocrine Society and SEEN do not endorse this mechanism: peripheral conversion to downstream steroids is unpredictable and depends on sex, age, tissue enzymes and existing pathology. Women face increased testosterone and estradiol with acne, hirsutism and menstrual disruption; men face gynecomastia and reduced libido. The concept of «hormonal balance» is not used in endocrinology: real disorders have specific names – hypothyroidism, hyperprolactinaemia, PCOS and so on – and receive targeted treatment. Hormonal complaints require labs and an endocrinologist visit, not pregnenolone from iHerb.

F

Anti-aging and longevity

Not recommended

Pregnenolone joins the biohacker master youth hormone stack alongside DHEA. There is no human anti-aging data: no long-term RCTs in healthy people with mortality or age-related disease endpoints have been done, and the only longevity finding is lifespan extension in the worm C. elegans, a model-organism surrogate, not clinical evidence. Pregnenolone is an active steroid precursor: it converts to androgens, estrogens, and cortisol, so uncontrolled use carries a real adverse profile (acne, hirsutism, hair loss, palpitations, insomnia, menstrual disruption) and is hazardous in hormone-dependent conditions (prostate and breast cancer, PCOS, thrombosis risk), with additive risk alongside HRT, combined contraceptives, DHEA, or testosterone. Benefit is unproven while harm is documented, which places it at F. What slows ageing is physical activity, a Mediterranean diet, smoking cessation, and blood-pressure and glucose control.

F

Memory and concentration enhancement

Not recommended

Pregnenolone is marketed as the «number one nootropic» and a «memory drug». No quality RCTs in healthy adults with memory and cognitive endpoints exist. The Moriguchi 2017 systematic review found only small trials in schizophrenia and bipolar disorder with conflicting results – not a basis for recommending the substance to healthy people. AEMPS and have not registered pregnenolone as a nootropic. Evidence-based options for memory and concentration are physical activity, sleep hygiene, BP and glucose control, learning and anxiety reduction. Memory complaints require in-person evaluation by a neurologist or general practitioner.

How to read tiers A–F
  • AStrong evidence – first or second line in international guidelines
  • BRecommended in selected cases
  • CStudy data contradict each other
  • DBenefit not proven, no documented harm
  • FDocumented harm, or risk without benefit

Mechanism of action

Pregnenolone is the master steroid: produced from cholesterol by CYP11A1 in mitochondria of adrenal cortex, gonads and glial neurons. Branched pathways yield progesterone, cortisol, aldosterone, DHEA, testosterone and estradiol. In the brain pregnenolone sulphate modulates GABA-A and NMDA receptors, which underlies the biohacker promise of «improved cognition and memory». Oral bioavailability is low; pharmacokinetics is poorly characterised. EU and regulators have not approved the substance as a drug for any indication.
