Incorporated into bone hydroxyapatite and taken up by osteoclasts during resorption. Inhibits farnesyl pyrophosphate synthase, disrupts protein modification, and induces osteoclast apoptosis. Compared with alendronate, more convenient dosing (35 mg weekly or 75 mg twice monthly or 150 mg monthly) and lower esophagitis rate. Effect persists for years – bone half-life is several years.
First line
First-line for postmenopausal osteoporosis per AACE/ACE 2020 and SEIOMM 2024 alongside alendronate. Cuts vertebral fracture risk by 41%, hip fracture by 39% (VERT). Standard dose 35 mg weekly. Reassess at 5 years (drug holiday). Also indicated in glucocorticoid-induced osteoporosis on long-term prednisolone 7.5 mg or more.

Risedronic acid is a second-generation bisphosphonate. It is prescribed for osteoporosis with confirmed reduced bone density, Paget's disease, and glucocorticoid-induced osteoporosis (NOGG 2021, AACE 2020). In anti-aging circles, risedronate is promoted for anti-aging bone strengthening in healthy perimenopausal women without diagnosed osteoporosis. In people without confirmed osteoporosis, no clinical studies of prophylactic use exist. The drug has serious adverse effects: esophageal erosions and ulcers (requires intake with plenty of water in upright position), osteonecrosis of the jaw, and atypical femoral fractures on long-term use. If risedronate was prescribed without confirmed osteoporosis, consider seeking a second opinion.

Risedronic acid is contraindicated in: Bisphosphonate hypersensitivity; Hypocalcemia; Esophageal abnormalities (achalasia, stricture) or inability to stay upright for 30 minutes; Severe renal impairment (eGFR under 30); Pregnancy and lactation. Full list in the Safety section.
