Assess acute pancreatitis severity with the 5 BISAP criteria: BUN, mental status, SIRS, age, pleural effusion.
BISAP is a simple bedside score, usable within 24 hours of admission, to estimate in-hospital mortality risk in acute pancreatitis. Five binary criteria, 1 point each: BUN >25 mg/dL, impaired mental status (GCS <15), SIRS (≥2 criteria), age >60, pleural effusion on imaging. Score ≥3 raises mortality 6- to 22-fold. BISAP is less precise than Ranson or APACHE II but needs fewer data and works in the ER.
