SOFA, Sequential Organ Failure Assessment
Six organ systems scored 0-4, maximum 24. Under Sepsis-3, sepsis is defined by a rise of 2 points from baseline.

About this calculator
The score rates respiration by PaO2/FiO2, coagulation by platelets, liver by bilirubin, haemodynamics by mean arterial pressure and vasopressors, consciousness by the Glasgow Coma Scale, and kidneys by creatinine or urine output. Each system contributes 0 to 4 points, up to 24 in total. Two parts of the scoring are not arithmetic. Respiratory scores of 3 and 4 apply only with mechanical ventilation or CPAP: without respiratory support the scale stops at two points however low the PaO2/FiO2 ratio is. The renal score takes the worse of creatinine and urine output rather than adding them. The key point about interpretation: since 2016 Sepsis-3 defines sepsis not by an absolute SOFA value but by a rise of 2 points or more from the patient's own baseline. In chronic kidney disease or cirrhosis the baseline SOFA is already above zero, and comparison must run against that starting point. The mortality band shown here comes from the 1996 derivation cohort and reflects the intensive care of its time, so it suits stratification rather than a prognosis for an individual.
