D-ribose has been studied in myoadenylate deaminase deficiency (AMPD1), a rare metabolic disorder with poor exercise tolerance. Wagner & Salerno (1991) reported reduced muscle weakness and cramping in a small patient group. D-ribose is also part of empirical «mitochondrial cocktails» for MELAS and other mitochondriopathies. Large RCTs are lacking; a metabolic specialist makes the decision individually. Standard support in most mitochondrial patients relies on CoQ10, riboflavin, L-carnitine and thiamine.
