Immunomodulator for disease-modifying therapy of multiple sclerosis

Dimethyl fumarate is converted to monomethyl fumarate after oral dosing – activates the Nrf2 transcription factor, triggers antioxidant response, and suppresses pro-inflammatory NF-κB signaling. Peripherally reduces cytotoxic T cells and pro-inflammatory monocytes. Exact mechanism in MS is incompletely understood. DEFINE and CONFIRM (NEJM 2012) – 44-53% reduction in annualized relapse rate vs placebo.
First line
Relapsing-remitting multiple sclerosis in adults. Per 2022 and SEN 2024 – moderate-efficacy disease-modifying therapy. Dose: 120 mg twice daily for 7 days, then 240 mg twice daily. Take with food – reduces flushing seen in 30-40% during the first weeks. Baseline: JC virus serology, CBC, lymphocytes, ALT.

Not recommended
Dimethyl fumarate is sometimes promoted in biohacking circles as an Nrf2 activator for anti-aging and detox. Nrf2 activation is described in vitro and in animal models, but clinical benefit in adults without multiple sclerosis or moderate psoriasis is not confirmed. In healthy adults it causes marked flushing, GI distress (diarrhea, nausea 30-40%), and lymphopenia with risk of progressive multifocal leukoencephalopathy (PML – rare but fatal). If prescribed for longevity without MS or moderate-severe psoriasis, consider getting a second opinion.
