Prednisone

AEMPS

Short-to-medium acting systemic glucocorticoid

ATC code: H02AB07

Brand names: Deltasone, Rayos

Updated · Methodology

Indications

A

Autoimmune conditions

First line

B

Rheumatoid arthritis

Adjunct

Glucocorticoid as bridge therapy at rheumatoid arthritis treatment start, while DMARDs (methotrexate, leflunomide) reach full effect. SER recommends short low-dose courses (prednisone ≤7.5 mg/day) tapered down over 3–6 months because of osteoporosis and metabolic complications.

ICD-10: M06.9 · ICD-11: FA20

How to read tiers A–F
  • AStrong evidence – first or second line in international guidelines
  • BRecommended in selected cases
  • CStudy data contradict each other
  • DBenefit not proven, no documented harm
  • FDocumented harm, or risk without benefit

Mechanism of action

Prodrug: liver-converted to active prednisolone via 11β-hydroxysteroid dehydrogenase type 1. Binds cytoplasmic glucocorticoid receptor, induces transcription of anti-inflammatory genes and suppresses NF-κB. Activity: 4× glucocorticoid, 0.8× mineralocorticoid vs cortisol. Half-life 2–4 hours, biological action 12–36 hours.

Safety

Contraindications

  • Hypersensitivity to prednisone

Serious adverse effects

  • Adrenal suppression and crisis on abrupt withdrawal
  • Severe infections due to immunosuppression
  • Psychosis
  • Avascular bone necrosis (especially femoral head)

  • Glaucoma and cataract with prolonged therapy

Common adverse effects

  • Weight gain and Cushingoid features
  • Insomnia and mood changes
  • Hypertension and sodium retention
  • Hyperglycaemia
  • Gastritis and peptic ulcers
  • Osteoporosis with prolonged therapy

Uncommon adverse effects

  • Adrenal suppression and crisis on abrupt withdrawal
  • Avascular bone necrosis (especially femoral head)
  • Glaucoma and cataract with prolonged therapy
  • Psychosis and mood lability
  • GI perforation
  • Proximal myopathy (especially at doses above 30 mg/day)
  • Latent infection reactivation (tuberculosis, hepatitis B, strongyloidosis)

Pregnancy: can you take Prednisone?

AEMPS summary of product characteristics (section 4.6): in pregnancy, treatment is started only after careful assessment of its risks and benefits, and the elimination half-life of glucocorticoids may be prolonged. In animal studies, prednisone and prednisolone caused cleft palate. Data on whether first-trimester glucocorticoid treatment increases the risk of oral clefts in the human fetus are limited. With prolonged treatment, intrauterine growth disorders cannot be excluded, and treatment near the end of pregnancy carries a risk of fetal adrenal cortex atrophy, which may require replacement therapy in the newborn with gradual tapering.

Breastfeeding: can you take Prednisone?

Compatible. Hale L2. Transfers into milk in small amounts. Single pulse doses and short courses do not produce clinically meaningful infant exposure. With prolonged high-dose therapy, monitor infant growth and adrenal function.

Reference information, not a clinical decision. Discuss feeding pauses or changes with your physician or an IBCLC.
