Silymarin (Silybum marianum)

Cochrane 2017 reviewed 18 randomised trials in 1088 patients with alcoholic or viral liver disease: milk thistle did not reduce all-cause mortality, cirrhosis complications or improve liver histology. EASL and AASLD do not recommend silymarin for alcoholic liver disease. The evidence-based intervention is full alcohol abstinence; in severe hepatitis, systemic glucocorticoids by the Maddrey score apply. Alcohol dependence is best addressed through programmes (psychotherapy, naltrexone, acamprosate), not a milk thistle course.

Milk thistle is often advised for «fatty liver» on ultrasound. Meta-analyses show weak and inconsistent effects on ALT and AST (5–10 IU/L reduction) and no change in steatosis grade on imaging. EASL 2024 and AASLD 2023 do not mention silymarin in MASLD/NAFLD management standards. Evidence-based options are 7–10% weight loss, Mediterranean diet, glucose and lipid control, and in confirmed MASH resmetirom or semaglutide as prescribed. Concerns about a fatty liver ultrasound finding require full assessment by a gastroenterologist, not a supplement course.

«Cleansing the liver of toxins» is a common marketing narrative around milk thistle, lemon water and «detox teas». The lay concept of toxins lacks an evidence-based definition: a healthy liver and kidneys clear the blood without supplements. AEMPS, EFSA and AESAN consider «detox» claims misleading. Concerns about general feeling require lab work and a physical exam, not a supplement «cleanse». Specific symptoms warrant a clinician visit.

Milk thistle was previously trialled as an adjunct in chronic hepatitis C. After direct-acting antivirals (sofosbuvir, velpatasvir, glecaprevir/pibrentasvir) achieved >95% cure rates, milk thistle lost any practical role. Cochrane 2017 and AASLD 2023 do not recommend it. Hepatitis C requires a hepatologist and a direct-acting regimen, not a supplement.

Milk thistle is mass-marketed as a «hangover pill» and «liver protection before drinking». No direct randomised trials of efficacy in hangover exist; theoretical grounds are thin because ethanol does not metabolise via OATP uptake. AEMPS and AASLD do not recommend the drug for this purpose. Evidence-based harm-reduction is drinking less alcohol, more water and adequate sleep. A severe or recurrent hangover is a reason to discuss alcohol use with a clinician, not to buy a supplement.
