Mechanism
Fluconazole prolongs QT (5-10 ms) + inhibits CYP3A4 and CYP2C9 (amiodarone metabolism). Dual risk.
Symptoms
QTc above 500 ms, syncope, torsades de pointes risk.
Management
ECG monitoring. If possible – terbinafine for superficial infections.
