MinorAntiplatelet agents – P2Y12 receptor inhibitors×Proton pump inhibitors

Updated · Methodology

Mechanism

Clopidogrel is a prodrug that needs CYP2C19 for activation, and its label lists the inhibitors to avoid: omeprazole and esomeprazole, fluvoxamine, fluoxetine, moclobemide, voriconazole, fluconazole, ticlopidine, carbamazepine and efavirenz. Pantoprazole is not on that list, which is why it serves as the compatible PPI when the stomach needs protection without losing antiplatelet effect.

Symptoms

The combination produces no symptoms of its own. What is watched is bleeding from the antiplatelet itself: black stools, bruising, gum bleeding. A long PPI course adds hypomagnesaemia and lower vitamin B12, unrelated to clopidogrel.

Management

If a patient on clopidogrel needs a proton pump inhibitor, pantoprazole is suitable: the clopidogrel SmPC approved by the European Medicines Agency (EMA) states that clopidogrel can be given with pantoprazole, and the pantoprazole label approved by the US Food and Drug Administration (FDA) requires no clopidogrel dose change. Omeprazole and esomeprazole are avoided with clopidogrel (clopidogrel SmPC, EMA; clopidogrel label, FDA).
