Ritonavir is the strongest CYP3A4 inhibitor. Sirolimus rises 15-fold.
Symptoms
Acute nephrotoxicity, thrombotic microangiopathy.
Management
Combination contraindicated. For HIV in transplant patient – ARV regimen without ritonavir or cobicistat.
