The drug is promoted for these uses outside international guidelines. Each entry below is analyzed against AEMPS, FDA, EMA, Cochrane and major RCTs.
- FCetylpyridiniumNot recommended · Cationic quaternary ammonium antiseptic for topical use in the mouth and throatSince 2020 cetylpyridinium has been sold as protection against coronavirus: a mouthwash or lozenge «kills the virus in the mouth». In a test tube a cationic antiseptic does destroy the lipid envelope of the virus, yet that gives a person no protection from infection. The 2020 Cochrane review of gargling and nasal sprays for healthcare workers during aerosol-generating procedures found emptiness on this question: there are no studies showing a reduction in infections. The WHO questions and answers on COVID-19 do not list gargling as a preventive method. Protection comes from vaccination, ventilation and a mask in crowded places, not from a throat lozenge. If a cetylpyridinium mouthwash was sold as COVID-19 prevention or as «clearing viruses», consider seeking a second opinion.
- FEthanolNot recommended · Skin antiseptic and disinfectantIn 2020 alcohol turned from a hand antiseptic into a «cure for the virus», with advice to drink spirits, gargle alcohol and spray it on oneself. The WHO questions and answers on COVID-19 answer directly: drinking alcohol does not protect against coronavirus, and spraying alcohol on skin and clothes does not kill a virus inside the body while it harms the eyes, mouth and airways. The AEMPS ficha técnica limits use to external: skin only, do not swallow, avoid contact with mucous membranes. The mechanism explains why alcohol is useless internally: it denatures proteins on direct contact, and such concentrations cannot be created in the bloodstream without poisoning the person. If alcohol was offered to drink, gargle or spray for protection from the virus, consider seeking a second opinion.
- FPovidone-iodineNot recommended · Broad-spectrum antiseptic, iodine productSince 2020 povidone-iodine has been sold as a gargle and nasal spray «against the virus»: in a test tube it does inactivate coronavirus within minutes. That gives a person no protection from infection. The 2020 Cochrane review of antimicrobial gargles and nasal sprays for healthcare workers during aerosol-generating procedures found not a single study showing a reduction in infections. The WHO questions and answers on COVID-19 do not mention gargling as a preventive method. The risk meanwhile is not zero: iodine is absorbed from mucosa, regular gargling affects thyroid function, and in pregnant and breastfeeding women the ficha técnica asks directly to avoid prolonged use on large surfaces. If povidone-iodine gargling was recommended for COVID-19 prevention, consider seeking a second opinion.
How to read tiers A–F
Evidence tiers belong to the drug-indication pair, not to the drug as a whole: the same substance can be A for one condition and F for another.
- AStrong evidence – first or second line in international guidelines
- BRecommended in selected cases
- CStudy data contradict each other
- DBenefit not proven, no documented harm
- FDocumented harm, or risk without benefit
