Oral antidiabetic for type 2 diabetes. The redGDPS 2024 algorithm positions pioglitazone as a third-line option after metformin and GLP-1 RA / SGLT-2i in insulin-resistant patients, particularly with concomitant non-alcoholic fatty liver disease. In Spanish practice it is used less than GLP-1 agonists because of weight gain, oedema, heart failure and fracture risk. PROactive 2005 showed a 16% reduction in a composite cardiovascular endpoint but no significant effect on the primary endpoint.
