Mechanism
Itraconazole is the strongest CYP3A4 inhibitor. Ergotamine rises 20+ fold, fatal ergotism with gangrene. FDA Ergomar Section 4.
Symptoms
Digital ischaemia, gangrene, stroke within hours.
Management
Combination contraindicated.
