SCORE2-Diabetes
10-year cardiovascular risk in type 2 diabetes at ages 40-69. Includes age at diagnosis, HbA1c and eGFR.
Updated · Methodology
For people with type 2 diabetes aged 40 to 69 without established atherosclerotic disease and without severe target organ damage. In those cases the risk counts as very high without any calculation.
About this calculator
The model extends SCORE2 with the three variables that shape prognosis most in someone with diabetes: age at diagnosis, HbA1c and estimated glomerular filtration rate. The logic is straightforward. Diabetes diagnosed at 30 has produced more years of hyperglycaemia by age fifty than diabetes diagnosed at 55, and with all other factors equal the risk differs markedly. Kidney function enters non-linearly, with a quadratic term on log eGFR: a fall from 90 to 60 weighs less than a fall from 45 to 30. Baseline survival and the regional rescaling factors are the same as in SCORE2; the whole difference sits in the coefficients. The limits of use are strict. The model covers ages 40 to 69 and type 2 diabetes only. It does not apply when established atherosclerotic cardiovascular disease or severe target organ damage is present: under the 2023 ESC guidance such a patient counts as very high risk without any calculation. Type 1 diabetes is not described by this model. Risk categories follow thresholds of 5, 10 and 20 %, and the category determines the LDL target.
