Several RCTs confirmed effects in acne and post-acne pigmentation. Clinical significance is comparable to plain salicylic acid with better tolerability. Does not replace retinoids or benzoyl peroxide as first-line therapy for severe acne.
Capryloyl Salicylic Acid (LHA – Lipohydroxy Acid) is a lipophilic salicylic acid derivative with a C8 caprylic chain attached. Patented and developed by L'Oréal in the 1990s (US Patent 5,262,407). Its lipid solubility lets it penetrate deeper into sebaceous gland openings and act more slowly than plain salicylic acid – less irritation, longer skin residence. Where applied. Anti-acne gels and toners, post-acne mark products, exfoliants for oily and combination skin. Concentration 0.1-0.5% (US allows up to 2%, EU up to 2% in face products). The flagship is La Roche-Posay Effaclar (toner, gel, concentrate). In Spain also SkinCeuticals LHA Cleanser, La Roche-Posay Effaclar Astringent Lotion. Mechanism. Keratolytic – dissolves bonds between stratum corneum corneocytes, accelerates desquamation. Unlike salicylic acid, it does not dissociate in aqueous phase (pH-independent) and works in the lipophilic environment of the sebaceous plug. Slower action, gentler exfoliation. Evidence base. L'Oréal studies (Uhoda 2003, Zheng 1998) showed a 20-30% reduction in comedone count over 6-8 weeks at 0.1-0.3%. Effect comparable to salicylic acid 2% with better tolerability. Parallel brightening effect on post-acne pigmentation – at 12 weeks. Safety. No standalone CIR report on LHA, but it belongs to the salicylate group with a well-known profile. EU SCCS – safe up to 2%. Irritation possible in 5-10% of patients during the first 2-3 weeks. Moderate photosensitization – SPF 30+ required. Pregnancy and lactation – use with caution. Topical salicylates (including salicylic acid up to 2% and LHA) are not formally contraindicated, but AAD 2024 and SEGO 2023 recommend avoiding them in pregnancy as a precaution. Alternatives – azelaic acid, niacinamide, glycolic acid 5-10%. Suitable for. Oily and combination skin, comedonal and mild inflammatory acne, post-acne marks, enlarged pores. Sensitive skin and rosacea – use with caution.
