Evidence base. The manufacturer (Pentapharm) reported a 50% reduction in wrinkle depth over 28 days in a 45-patient study at 4% SYN-AKE. Independent RCTs with comparable design are scarce, and effect is more modest than botulinum-toxin injections. 'SYN-AKE equals botox' comparisons are incorrect. Safety. CIR confirmed safety at cosmetic concentrations. Non-sensitizing, non-comedogenic. No systemic absorption. Pregnancy and lactation – data are limited. Because of theoretical neuroactivity manufacturers label as 'not for pregnancy', although topical systemic absorption is zero. Alternatives – niacinamide, adenosine, Matrixyl peptide complexes.
