Mechanism
Fluvoxamine is the strongest clinical CYP1A2 inhibitor. Theophylline rises 3-fold. Fatal cases.
Symptoms
Seizures, arrhythmias within 3-7 days.
Management
Combination contraindicated. Alternative – sertraline or escitalopram (no CYP1A2 inhibition).
