Mechanism
Posaconazole is a strong CYP3A4 inhibitor. Simvastatin rises 10+ fold, rhabdomyolysis risk. EMA Noxafil Section 4.3.
Symptoms
Rhabdomyolysis within 5–10 days.
Management
Combination contraindicated. Pravastatin or rosuvastatin.
