Early studies show antioxidant protection of skin cells from UV stress. Large comparative RCTs are few, so reviews classify idebenone among «promising but not proven» antioxidants.
Idebenone is a synthetic ubiquinone (CoQ10) analog with a shortened side chain that makes it more stable and water-soluble. Originally developed in Japan (Takeda Pharmaceutical) for neurodegenerative disease – oral form treats Friedreich's ataxia, Leber's disease, MELAS syndrome. Came to cosmetics in the early 2000s as a "next-generation" antioxidant. Mechanism. Inside mitochondria substitutes for deficient ubiquinone in the electron transport chain, protects membranes from oxidative stress, neutralizes free radicals better than vitamin E, CoQ10, and α-lipoic acid (Mc Daniel 2005 – Environmental Protection Factor). UV-stable, does not degrade in light. Where applied. Anti-aging serums and creams (0.5-1%). In Spain – Allergan line (formerly Prevage MD, then sold to Elizabeth Arden), Heliocare 360° Idebenone (Cantabria Labs). Often in premium segment due to high raw material cost (1 kg ~1500-2000 EUR). Evidence base. McDaniel 2005 (41 women, 6 weeks, 1% idebenone) – 33% reduction in skin roughness, 27% reduction in fine wrinkles. Sjuts 2007 – improved skin microrelief. RCTs are relatively small and mostly sponsored by the manufacturer. Safety. CIR rated idebenone safe at cosmetic concentrations (2010). Irritation and allergic reactions are rare (1-3%), but allergic contact dermatitis has been reported (Sasseville 2007). Pregnancy and lactation – topical at cosmetic concentrations is considered safe (minimal systemic absorption), but no direct data. Systemic (oral) use in pregnancy is contraindicated.
