Topical application has no clinically confirmed efficacy due to the inability of these large molecules to cross the epidermal barrier. PDRN creams may provide surface moisturization as formula excipients, but not through specific PDRN activity.

Intradermal PDRN injections for photoaging and atrophic scars are supported by several RCTs and prospective studies. Improvements in skin hydration, tone, and dermal density have been demonstrated. Allergic potential is minimal – salmon protein is absent in purified formulations.
Polydeoxyribonucleotide (PDRN) is a mixture of low-molecular-weight DNA fragments (50–1500 base pairs) obtained from salmon or trout sperm. In pharmaceuticals used in injectable products for skin and cartilage repair (Placentex in Italy, Rejuran in Korea). In cosmetics – a premium anti-aging component in Korea and Japan. Mechanism. Activates A2A adenosine receptors on fibroblasts and keratinocytes, stimulates cell proliferation, type I and III collagen synthesis, hyaluronic acid. Enhances microcirculation via VEGF. Anti-inflammatory effect. Where applied. Injectable products for bio-revitalization and post-procedure recovery (Rejuran, Placentex – prescription via dermatologists and aesthetic doctors). In cosmetics – premium Korean serums and creams (Rejuran Healer, Donginbi). In Europe and Spain rare in cosmetic form; the injectable form is available in aesthetic clinics. Cosmetic concentration 0.1–1%. Evidence base. Galeano 2008 RCT in wounds confirmed accelerated healing with injectable PDRN. Korean RCTs Lee 2019 showed wrinkle depth reduction and texture improvement over 12 weeks with 1% PDRN in cosmetics. No systematic review of topical use. Safety. Contact allergies are rare. In patients with fish allergy – contraindication (theoretical risk). With injectable use – local reaction (swelling, redness) in 5–10% of patients. Pregnancy and lactation – no data. Manufacturers recommend avoidance as a new component with limited clinical base. Alternatives – adenosine, peptides, niacinamide with stronger evidence.
