Anti-inflammatory properties of bisabolol confirmed in small clinical studies for dermatitis and eczema. Whole extract efficacy is hard to standardize. No large RCTs comparing with topical corticosteroids.
Chamomile Extract is a general INCI for water, glycol or alcohol extracts from Matricaria chamomilla (German chamomile) or Anthemis nobilis (Roman chamomile) flowers. Formulas usually use German chamomile. Active components: bisabolol (5-20%), chamazulene, flavonoids (apigenin, luteolin), coumarins. Where applied. Soothing creams and serums, toners, masks, sensitive-skin and eye-area products. Extract concentration 0.1-5%. In Spain – Klorane, Avène Pediatril, ISDIN Baby Naturals, Mustela. Mechanism. Bisabolol inhibits COX-2 and 5-LOX, lowering skin prostaglandin and leukotriene synthesis. Chamazulene gives antioxidant effect. Apigenin blocks NF-κB signaling. Combined – mild anti-inflammatory effect, comparable to 0.5% hydrocortisone on visual redness (Patzelt 2012), but without action on edema and itch. Evidence base. The Cochrane 2017 review on topical agents in atopic dermatitis: chamomile showed moderate effect in mild disease, insufficient in moderate-severe. ESCOP 2003 and EMA HMPC 2015 recognize chamomile as a traditional-use agent for skin and mucosal irritation. Safety. CIR confirmed safety at cosmetic concentrations. Allergic reactions in 2-5% of pollen-sensitive patients to Asteraceae (ragweed, mugwort, cornflower). Cross-reactivity in 0.5-1% of the general population. Contact dermatitis is rare, more often to the essential oil than to glycolic extract. Child safety controversy. AAD 2024 and Spanish AEDV 2023 warn of contact-sensitization risk in atopic children. Alternatives in children – colloidal oatmeal, panthenol, centella. Pregnancy and lactation – safe topically. Acceptable at any stage. Oral chamomile tea in pregnancy per EMA HMPC 2015 is not contraindicated, but in large doses may theoretically trigger uterine contractions. Suitable for. Mild skin irritation, post-procedure redness, sensitive adult skin. In atopic children and pollen-sensitive patients – use with caution.
