Cyanocobalamin is a synthetic vitamin B12 form with a cyano group. In skincare – an ingredient with anti-inflammatory and brightening potential; in systemic medicine – replacement therapy for B12 deficiency. Mechanism. In skin vitamin B12 participates in DNA methylation, myelin synthesis (for peripheral nerves), and inflammation regulation via NF-κB suppression. Topical penetration through the stratum corneum is limited – the molecule is water-soluble and large (1355 Da). Where applied. Creams for atopic dermatitis patients, sensitive-skin formulas, niche Japanese and Korean lines. In Spain – Avène TriXera, La Roche-Posay Lipikar within multi-component formulas. The pink color of cyanocobalamin (a crystal property) gives a characteristic tint to creams without added dyes. Evidence base. Stuker 2009 open-label study: 0.07% cyanocobalamin cream over 8 weeks reduced symptom severity in atopic dermatitis patients vs the base – 50% SCORAD improvement. Meta-analyses and larger RCTs haven't confirmed clinical significance; the effect remains moderate. Safety. CIR confirmed Cyanocobalamin safe in cosmetic use. Allergic reactions are extremely rare. Systemic absorption through intact skin is low. Pregnancy and lactation – safe. B12 is needed in pregnancy and breastfeeding; replacement systemic doses for deficiency are prescribed freely. Topical cosmetic concentrations don't create issues. Best suited to: atopic dermatitis, sensitive inflamed skin. Not a first-line active – for atopy a hypoallergenic emollient with ceramides and colloidal oatmeal is more reliable.
