Dimethyl Sulfone (MSM) is a sulfur-containing compound, the end-oxidation product of dimethyl sulfoxide (DMSO). Known as methylsulfonylmethane in dietary supplements; entered cosmetics with an 'organic sulfur source' marketing angle. Mechanism. Sulfur is a structural component of keratin, collagen, elastin, and glutathione (a thiol antioxidant). Manufacturers market MSM as 'building material' for skin and hair. In lab models it suppresses pro-inflammatory cytokines and NF-κB activity. On real skin penetration and efficacy are limited – the molecule is water-soluble and small, but active dermal effect isn't characterized. Where applied. Anti-inflammatory creams and serums, 'sulfur boost' hair products, niche anti-acne and rosacea products. In Spain – Sukin, Nature's Plus, niche artisan brands with 'sulfur for skin' marketing. Evidence base. Oral MSM joint benefit data in RCTs are mixed. Topical skin clinical data are scarce. Berardesca 2008 open-label study showed moderate symptom improvement in rosacea patients with 1% MSM in a cream – result not confirmed in large placebo-controlled RCTs. Safety. CIR confirmed Dimethyl Sulfone safe in cosmetic use. Hypoallergenic, non-comedogenic. Allergic reactions rare. In patients with sulfonamide allergy reactions are theoretically possible, extremely rare in practice. Pregnancy and lactation – safe at standard cosmetic concentrations. Best suited to: sensitive inflamed skin within a multi-component formula. Not a first-line active – evidence base is limited.
