Lithospermum Erythrorhizon Root Extract (Chinese herb Zicao) is a root extract of a Boraginaceae plant. Active components are shikonin and its esters (acetylshikonin, deoxyshikonin). Used in traditional Chinese medicine for over 1500 years for burns and skin inflammation. Mechanism. Shikonin suppresses NF-κB and reduces IL-6, TNF-α production in keratinocytes in vitro (Andújar 2013). Provides anti-inflammatory and antibacterial effects against S. aureus and Cutibacterium acnes. Where applied. Korean and Japanese sensitive-skin and anti-irritation products, a niche in premium cosmetics. Rare in Europe. Formula concentrations 0.1–1%. Evidence base. Human RCTs are few (n<30), all from Korea and Japan. No systematic review. CIR and SCCS have not published a separate assessment. Pregnancy and lactation – use with caution. Shikonin and pyrrolizidine alkaloid esters may theoretically cause hepatotoxicity on systemic absorption. Topical use at 0.1–1% is rated as conditionally acceptable, but direct data are absent – manufacturers advise avoidance.
