Topical application

The effect stays symptomatic and does not cure rosacea, only removes redness temporarily. A common issue is rebound erythema after withdrawal or overdose.
Brimonidine Topical is a selective alpha-2-adrenoceptor agonist supplied as a 0.33% gel (Mirvaso, Onreltea). Approved for persistent erythema in adult rosacea. Mechanism. Binds α2 receptors on the smooth muscle of superficial facial skin vessels and triggers vasoconstriction. The effect is fast: visible redness reduction within 30 minutes, peak at 3–6 hours, duration 9–12 hours. Does not act on inflammatory rosacea lesions (papules, pustules) – erythema only. Where applied. Prescription 0.33% gel (Mirvaso, available in Spanish pharmacies), once daily in a thin layer on cheeks, nose, forehead, chin. Before an event or daily for persistent erythema. Do not apply to inflamed lesions, broken skin, or the perioral or periorbital area. Evidence base. Fowler 2012 (n=553, 12 weeks, RCT) showed significant erythema reduction on the CEA scale with 0.33% brimonidine vs placebo. Effect on erythema is fast and predictable. Long-term Layton 2018 (52 weeks) confirmed sustained activity. Adverse effects. Paradoxical rebound erythema 6–12 hours post-application – in 20–25% of patients in early weeks. Rarer with long-term use. Allergic contact dermatitis in 2–5%. Transient burning and stinging on application. With daily use over large areas effect wanes – tachyphylaxis. Pregnancy and lactation – use with caution. FDA category B, but systemic brimonidine (eye drops in glaucoma) enters breast milk. Topical absorption on limited areas is low, but AEMPS and SEGO recommend avoiding without necessity. For gestational rosacea use azelaic acid, gentle care, and SPF. Particularly suitable for. Persistent erythema in rosacea without marked inflammation, episodic telangiectasia before an event. Not for patients with active papules/pustules, perioral dermatitis, untreated demodex. A dermatology consult is mandatory before starting.
